TELEHEALTH CONSENT

Timeless Aesthetics & Laser Clinic and/or its affiliates or subsidiaries (collectively, the “Clinic,” “we,” or “us”) own and operate our website, patient portals, and any related digital platforms or applications used to provide certain telehealth services (collectively, the “Service”). Your access to and use of the Service requires your consent to receive care via telehealth.

The purpose of this Telehealth Consent (“Consent”) is to inform you about telehealth services and to obtain your informed consent for the use of telehealth in delivering services to you by licensed healthcare providers affiliated with Timeless Aesthetics & Laser Clinic (“Providers”).

Throughout this Consent, the terms “you” and “your” refer to the individual using the Service.

By clicking “I Agree,” checking any box signifying acceptance, completing electronic consent, or using telehealth services through the Service, you acknowledge that you have read, understood, and agree to be bound by this Consent.

If you do not agree, do not create an account or use the Service.

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a Provider and a patient who are not in the same physical location.

Telehealth services may be used for medical evaluation, diagnosis, treatment, follow-up care, patient education, and care coordination, and may include, but are not limited to:

• Electronic transmission of medical records, photographs, personal health information, or other data
   
• Audio, video, or secure messaging interactions between you and your Provider
   
• Use of data transmitted from medical devices, images, sound, or video files
   

As with any medical service, there are potential risks associated with telehealth, including but not limited to:

• Limitations in the quality, accuracy, or effectiveness of services due to the absence of an in-person examination
   
• Technical failures, bugs, interruptions, or inaccuracies within the technology or Service
   
• Inability of a Provider to offer treatment for certain conditions via telehealth
   
• Failures of electronic equipment or connectivity
   
• Security breaches or unauthorized access to electronically stored or transmitted information
   
• Increased risk that protected health information may be unintentionally disclosed
   

The electronic systems used by Timeless Aesthetics & Laser Clinic incorporate administrative, technical, and physical safeguards designed to protect the privacy and security of your information.

You understand that communications may occur via secure platforms, email, or other electronic means that may include protected health information. While reasonable safeguards are in place, the Clinic cannot guarantee the absolute security or privacy of electronic communications, including those transmitted through third-party email or internet services.

• You understand that telehealth is voluntary and that you may withdraw your consent at any time by providing written notice to the Clinic
   
• Because services are provided exclusively via telehealth, withdrawal of consent will require you to seek care from another provider
   
• Withdrawal of consent will not affect prior services already rendered
   
• All other terms of this Consent remain in effect regardless of withdrawal
